What ROSM Can Do For You
Social service agencies can submit a ROSM REQUEST — a request distributed to participating religious organizations asking them to provide financial assistance for a client in need. ROSM coordinates the response and notifies you of the results.
Criteria for Submitting a ROSM Request
- Along with all request information, we must have a signed statement of release of information from the client.
- Only one request per calendar year per individual.
- No requests for deposits.
- No funeral expenses.
- No lodging requests, except in exceptional circumstances where the client does not meet the criteria of existing shelters.
- No telephone bill requests, except in cases of health or safety.
If you meet the criteria above, submit your request with verified information along with the statement of release of information to: rosmoffice@rosmky.org or fax to 685-3245.
Required Information for Each Request
- Your contact information (telephone and email address)
- Client's full name (spelled correctly), address, and telephone number
- Last 4 digits of Social Security number and date of birth for client (if client has no SS#, ROSM will assign a number)
- Monthly income information
- Other adults (anyone over age 18) in the home — include relationship to client, last 4 digits of SS#, and date of birth for each
- Number of children in the home and date of birth for each
- Existing need: type, amount, and due date
- Payment information:
- For rent requests: landlord's name and telephone number. Before contacting ROSM, confirm with the landlord whether payment will satisfy the debt or prevent eviction.
- For utility requests: account number
- Narrative describing the client's circumstances and why you feel this client is worthy of assistance
- What plan is in place to pay next month's bills
What Happens Next?
Your request will be checked against the ROSM database. If your client meets the criteria, a "ROSM REQUEST" will be sent via email to participating religious organizations. If you have email, you will receive a copy.
You will be notified within one week regarding the results. The religious organization that receives the request may contact you with additional questions.
If they can assist, religious organizations will make payment directly to the debt or bill, and will inform ROSM of their assistance. ROSM will then notify you of any assistance provided.
If your request for assistance is denied, please contact the ROSM coordinator for an explanation.
